NEWS FLASH!!! (11.05.2020) - Update following the announcement regarding the closure of St Andrew's Surgery during the current pandemic. We are now looking to offer a small number of pre-booked Nurse appointments. These will mainly be offered to vulnerable patients, but will include some routine procedures moving forward.

Depression Questionnaire

Depression or other mental health problems will affect a significant number of us at any time in our  life.  It can be difficult to talk about this but if the problem is identified there are lots of different ways we can help.

It is quite normal for your mood to fluctuate day to day especially during times of stress or upset, but if you feel your mood has been persistantly low and that you might be depressed this easy to use patient questionnaire may prompt you to seek help.

It is used by your doctor to monitor the severity of depression and response to treatment.
